I am a Ph.D. student at Simmons University and Assistant Director at
Minitex, University of Minnesota Libraries. I would like to invite you to
participate in my research study designed to elicit a better understanding
of leadership perceptions of librarians in academic libraries. My intent is
to analyze responses so that a clearer picture and understanding of
librarians currently working in academic libraries and their leadership
perceptions emerges.

The first question is an Informed Consent statement with options for you to
indicate “Yes, agree to consent” or “No, don’t agree to consent”. Assuming
you agree, the survey will continue for you to complete and estimated
completion time is 10-15 minutes, with completion on any computer or mobile
device. In the event you do not consent, the survey will end.
