Survey regarding Reorganization in Academic Libraries -- participation request (Please pardon cross-posting!)

Greetings!   The purpose of the attached survey is to collect data about recent and current trends in academic library reorganization.   The results will be reported in an upcoming issue of Serials Review.   With this survey I hope to obtain and share a broader understanding of the various reasons, scopes, methods and outcomes of library reorganization.   I am focusing primarily on academic libraries primarily for the purpose of having a manageable data population.   This study may be expanded to other library types in the near future.   Please complete and submit the survey by February 8, 2019.   Your input is highly appreciated!
