I'm an Italian academic librarian.
We ask the patrons to replace the lost or damaged material with a new 
item of the same edition.
If this is not possible (for it is unavailable but this case is very 
very rare) we ask to buy a new item of similar commercial / cultural value.
This is a good stategy for our system but it's time consuming because 
you need to follow the whole replacement procedure and, eventually, for 
out of print materials, to direct the patrons to the most appropriate 
purchase channels.
