My name is Alma Ortega and I am a student in the School of Leadership and
Education Sciences at the University of San Diego. I am conducting a
research study on leadership styles in academic libraries. This study is an
attempt to learn about leaders’ styles in academic libraries.

You are invited to participate in the study by answering multiple choice
and short answer questions in an online survey. Your opinions are valuable.
Your participation in this survey is completely voluntary, and you may
change your mind and stop at any time.

The survey can take from 12 to 20 minutes to complete. Your answers will be
saved anonymously. If you choose to provide contact information at the end
of the survey to receive the study’s results your information will be kept
strictly confidential. I may publish the results of the survey in a
publication or a presentation at a professional organization but no
identifying information will be included as it will be presented in the
aggregate. If you would like information or have any questions regarding
the survey please contact me by e-mail at alma_at_sandiego.edu.
