CDL-"Tracker" for database usage statistics

Hi all,

I am wondering whether any libraries out there have developed a "tracking 
sheet" for database usage statistics. I want to develop a similar checklist 
for our library, a list that will track where to go for a particular 
database's usage statistics, what sorts of data is available, how often the 
data is available, how the data is delivered, administrative passwords, etc.

A couple of years ago I went to a preconference on developing such an 
internal system, one that could be done in an Excel spreadsheet. Now I am 
tasked with developing said checklist. (Oh how those preconferences come 
back to haunt you . . . . :-) )

If any of you have experience with developing and using this type of 
system, I would welcome hearing from you. If you have other ideas on how to 
do this, I'd welcome hearing about those as well.

Basically, my goal here is to be able to lay my hands on the statistics 
when we need them for database evaluation, without having to constantly 
retrieve and collate. Wishful thinking perhaps, but there you have it.
