I'm interested in seeing the form as well when you have time. I'm looking for a way to communicate to faculty liaisons the data that we can offer using an easy to understand format. This sounds like a great way to get the word out and not be overwhelmed with email exchanges.

Our team has created request forms that liaison librarians use to request statistical, holdings, and downloads information.  These requests are ported to the appropriate staff member.  This team works closely with the Access Services team to list holdings for materials available for ILL, particularly Rapid.
