Are any public libraries using ILLiad in this manner or using ILLiad at all? We have been struggling with the same issue.

We also use ILLiad for this purpose, but we downloaded a few add-ons from the IDS Project to help. Check it out here: http://www.gistlibrary.org/illiad/#.V3Kiu_krJhE. I highly recommend this for purchase suggestions. It has streamlined my process!

We use Illiad for this purpose. We have a note field in the ILL request form that allows faculty & students to suggest a purchase. This is especially great because patrons don't have to know about two different forms. Our collection development librarian reviews all the requests in the loan queue and will purchase or ILL depending on several factors (cost, currency, interdisciplinarity, etc.). Then, all she has to do is route the request to an ILL queue or an acquisitions queue. No emails involved.
Often, people will request items that haven't been published yet and this is great for managing those because we can just move those into a future purchase queue and come back to them when they become available.

Are any of you using Springshare's LibAnswers to track and organize new book purchase requests? If you are, or have tried to but decided not to, could you please share your experience or recommendation?

At UNR, we are currently using Springshare's LibWizard-Forms for book purchase requests (http://guides.library.unr.edu/libacq/bookrequest). We also accept direct email requests from subject librarians. All of these requests go to our Acquisitions staff via email. Since we heavily rely on patron/librarian requests for books to firm order, we receive many hundreds of book request emails in a year. The high volume of book requests is making our current system unsustainable. In addition to LibAnswers, we have also considered possible homegrown options like Microsoft Access and SharePoint List function.
We also use Springshare's LibWizard-Forms for media (DVDs, streaming) purchase requests (http://guides.library.unr.edu/libacq/mediarequest). This form is working well for us so far. The lower volume of media requests we receive makes it much more sustainable than the book form.
