We only have to get this filled out every few years or so.  Each time the IRS makes significant changes to the form, our accounting department asks for new forms from our vendors.

Thanks to everyone who replied.  It's good to know that we are not alone in this!

I have one question; does this W8-ben need to be filled out only the *first* time by the foreign vendor, or *each* time you make a purchase from said vendor? Thanks.
