Yes, at the University of Hawaii at Manoa both the Serials and Acquisitions
departments are asked to send this form to our foreign vendors. Our vendors
are located throughout the Pacific and Asia, and the communication can be
very tricky. The form is daunting, and we cannot provide help to them when
filling it out (obviously) but the choices that they make impact our
purchase price immensely. We've had a few vendors fill out the form in ways
that changed their tax status in ways they were not expecting.
