For a commissioned article I am writing, I would appreciate feedback about your experiences re the following, 
especially if you are on staff at a special collections or rare book library or archives: 

(1) Does your institution have in place a WRITTEN policy regarding deaccessioning? 
(2) Is this policy well-publicized to staff, potential donors, your institution's administration and the general public? 
(3) Are there instances when this policy has been contravened? 
(4) What were the consequences of this contravention? 
(5) What, if any, changes have been made to your institution's deaccessioning policy in consequence of such 
contravention?
