Our library also allows patrons to bring in the exact book 
they lost and we waive the replacement cost but still charge 
the processing fee of $15. For others who do not want to buy 
an exact replacement, we check BIP to get the current selling 
price and then add $15 for processing. Many of the books 
from our library are irreplaceable, so for ones not found in 
BIP, we charge $75 plus the $15 processing fee. That normally 
gets them digging for the book and bringing it back.

We keep the retail price for all items, or a default 
amount for everything pre-computer, in our ILS and we 
charge our patrons that plus a $5 processing fee.

Here at the University of the West Indies, St. 
Augustine Campus. We use the cost of the book and 
a replacement fee. Usually we take the price
from Blackwell's or BIP.

We used to charge a flat replacement fee plus a $15 
processing fee.  That resulted in people complaining that 
they were paying too much for cheap paperback versions.  
We changed our flat-cost policy when a very expensive
book was lost and the flat rate didn't come anywhere close 
to covering the replacement costs. 

We dropped the flat-cost policy and now look up a replacement 
cost and tack on the processing fee.  It's a bit more work, 
especially if a title is out of print, but it's fairer all 
the way around.  We also allow the person who lost the item 
to buy his own copy as long as it's an acceptable edition and
in good condition. In those cases, we still charge the 
processing fee.

We have a standard fee of $81.00 plus a $5 processing fee.  We allow 
patrons to purchase a
replacement copy IF it is an exact replacement.  However, most patrons 
don't understand
the concept of edition, so we are leery of letting patrons purchase a 
replacement.

We charge cost of the book (using Amazon or bookfinder) 
+ $25 processing fee. We have a $45 minimum (which means 
we never charge a patron less than $45 total).  Our policy 
is to not allow patrons to purchase a lost book and then just 
give us the purchased copy, but, as always, we've
made a few exceptions (particularly for faculty).

University of Iowa charges $70 for lost/ruined materials, 
based on the $68.36 average price from our primary book 
vendor, YBP.  There's no processing fee, but any fines 
accrued still apply.

At both universities that I have worked at, one as head of access
services, there is a standard fee that is charged for replacement of
library materials-$50.00  In case the person finds the material after
paying a $10.00 processing fee is charged.  The rationale is that you
lose money with some books and gain with others.

We have an initial $60 default on their notice to get their attention so
they'll come in to talk to us. At that point we'll get the true price,
assuming it's in print, so they can make arrangements to pay. If the book is
truly out of print--no used copies available for purchase--we charge the $60
default. Sometimes we tell them to keep looking for the book. 9 times out of
10 they find it. You should also know we don't charge overdue fines. We
found it really made no difference in the timely return of books.

Standard fee, usually no. Processing fee, yes. We attach a $10
non-refundable fee to the actual replacement price. Our policy is if they
find the book within 6 months of paying, then they should return the book
and we'll refund all but the processing fee.

Yes. We use all these plus our usual vendors to shop around for the best
price for students, which includes tax and s/h (if any).

Not any more. Too often they purchased the wrong edition (i.e. abridged, 2nd
ed. when they lost the 4th, etc.) or the cheapest used copy they could find
(often in a condition we would not accept as a gift) or the wrong title. We
tell them we have vendor discounts and we will apply them to their benefit.
Sometimes faculty members will bring a replacement and we generally don't
argue with them if it's a new copy.
