Aloha from the other side of the island!
We recently did a study on adding a special collection of books and
media for a campus program that got a grant. We calcuated a shelf ready
cost to include OCLC (sometimes we already have a record), book plate,
lamination/Easy Cover, staff time (student employees), labeling,
barcode, tattle tape, book tape, media case thusly:
Paperback: $6.04
Hardback: $4.08
VHS $5.74
DVD $5.54
CD/CD-ROM $5.54

Subject: RE - Desperately Seeking Processing Costs
When a book is damaged or lost, we charge a replacement fee of $58. This
is for an academic hardbound title. If the book is subsequently
returned, the patron is refunded $48 minus a $10 processing fee. We have
only recently begun this policy and have yet to see how students react.
Formerly, we charged the cost of the book as entered in the
bibliographic record at the time of purchase. Also, this replacement
cost is now being returned to the library book budget whereas before the
money went into the general fund. We do not charge overdue fines.
